Health Requirements and Exclusions
Children attending school, pre-K through 12th grade, in New York State must receive all required doses of vaccines on the recommended schedule within the first 14 days of school in order to attend or remain in school.
By September 19, 2024, parents must:
Show proof of their child's up-to-date vaccinations.
Additionally, parents/guardians must submit a
school health examination (or physical) within 30 days of the start of school. School physicals are required for grades PK or K, 1, 3, 5, 7, 9, and 11. Physicals must be done by a New York State provider and documented on the state’s required Health Examination Form.

Parents/guardians of students that need to update their medical record will be notified by the Health Office.
