New in 2024-2025! ThriveWell Digital Health Platform
As of July 1, 2024, the Healthy Rewards Program for Elmira CSD subscribers and their spouses on the Excellus BlueCross BlueShield health plan is now hosted through ThriveWell, our new digital platform for health and well-being. ThriveWell provides access to tools and support to help you make small, everyday improvements to your overall well-being. Subscribers and spouses can earn up to $1,000 (up to $500 each) by participating in various activities on the platform between 7/1/24 and 6/30/25.
We’re excited to kick off the new school year by promoting ThriveWell and encouraging members to log in and engage with the platform to support their well-being journey. As part of this, we’re launching a Health Risk Assessment (HRA) incentive campaign! The HRA is an NCQA-certified, evidence-based self-assessment designed to help members understand their health risks and provide personalized recommendations for lasting improvements.
Starting 7/1/24, when members complete the Health Check (HRA) through ThriveWell, they’ll automatically earn a $25 reward. Additionally, anyone who completes the Health Check between 7/1/24 and 10/31/24 will be entered into a raffle for prizes, including a custom ECSD water bottle, a 5-pack of resistance bands, a yoga mat, or a stylish journal.
*Please note that those who completed the Health Check between 1/1/24 and 6/30/24 will need to retake the survey to qualify for the $25 reward and be entered into the raffle.
Elmira subscribers should look in their mail for upcoming announcements and information, encouraging them to log in to ThriveWell and complete the Health Check by 10/31.
