Student Physicals and Immunizations

Update from the Office of Health Services as of September 13, 2022 – 4:00 p.m.

Dear Elmira City School District families,

We are writing with important reminders on immunizations and school physicals. New York State has resumed the requirements for physicals and immunizations for the 2022-23 school year. Students must have all state-required immunizations within 14 days of the start of school. Immunizations need to be up-to-date (or an appointment scheduled) in order for students to attend school beginning Tuesday, September 20, 2022.

We are here to help. Below is a list of options in the case your student needs required immunizations:

1. Contact your students’ primary care provider for an appointment.

2. Participate in ongoing Chemung County Health Department Public Health Clinics:

Children’s Vaccine Clinic at the Ernie Davis Community Center (EOP)

• Thursday, September 15, 2022
• 1 p.m. – 6 p.m.
• EOP - Ernie Davis Community Center located at 650 Baldwin Street in Elmira
• To make an appointment, call the Chemung County Health Department at 607.737.2488

Chemung County Health Department, vaccination by appointment:

• To make an appointment, call the Chemung County Health Department at 607.737.2488
• Monday – Friday from 9 a.m. – 4 p.m.
• Vaccine clinics held on Fridays from 10 a.m. - 2 p.m. by appointment

Additionally, parents/guardians must submit a school health examination (or physical) within 30 days of the start of school. School physicals are required for grades PK or K, 1, 3, 5, 7, 9, and 11. The state’s Health Examination Form can be found here: Required New York State School Health Examination Form.

If you have any additional questions, please contact your student’s school nurse or call Melissa Montopoli at 607-735.3222.

Thank you and be well,
