Planning a field trip 

All trips outside the school require careful planning and communication well in advance of the actual trip. Plans must include:

• A signed parent permission form.
• Medical accommodations for students with special conditions or medication needs.
• Emergency contact information for every student.
• An approved chaperone list. Chaperones are approved by your building Principal.
• A cell phone for each teacher and chaperone on the trip. These numbers must be provided to the main office at the home school or the Athletic Office and to all adults on the trip.
• Information cards that students carry at all times while on the trip. These cards must include the student's name, school name and phone number, the cell phone number of his or her teacher/chaperone and information regarding medical conditions.

Day trips closer than 50 miles one way must be approved by your Principal. Submit a Field Trip Request Form at least two weeks in advance.

Trips that are overnight, outside a 50 mile radius or to a business owned by a member of the Board of Education must also be approved by the Superintendent and Board of Education. Submit this Field Trip Request Form to your Principal at least four weeks in advance.
