Welcome to "Connections"! 


What is transition?

It is a process that requires planning for a young adult's future. Connections will help ensure that advance planning for adult services and coordination begin before students exit school. Coordination of transition services should help students prepare for his/her future dreams.

Why is transition planning important?

Students often have a difficult time deciding what they want to do and knowing what they will be able to do once their high school years are completed. Students move from a supportive and familiar environment to a world of adulthood based on differing requirements and expectations. Transition Planning can make this change easier. 


What is transition planning?

Transition Planning is a partnership and a process. The "Transition Team" facilitates a cooperative effort among many people (school, home and community) to increase the success of the student and to help students:

- Link their interests with their strengths

- Apply their strengths to future endeavors"    

- Identify community service
